Welcome to the fitness planner, and congratulations -- you've decided to take a step toward your healthier future. This planner uses ideas from fitness experts to help you create a five-day exercise plan, customized to meet your goals. It also analyzes your weight and height to tell you how you compare to nationally recognized guidelines. Just answer the questions below (be honest!) for your personal exercise and eating plan.

Before we go on, please answer the following background questions that could determine whether you're at risk in pursuing an exercise program:

Your doctor said you have heart trouble, a heart murmur, a heart condition or you have had a heart attack.
You feel pain or pressure in the chest area during or right after you exercise.
You experience extreme breathlessness after mild exertion.
Your doctor said your blood pressure was too high, and/or it is not under control. Or you don't know whether or not your blood pressure is normal.
Your doctor is currently prescribing drugs for your blood pressure or heart condition.
Your doctor said you have bone or joint problems such as arthritis.
You are over age 60 and not accustomed to vigorous exercise.
You lose your balance because of dizziness.
Your father, mother, brother or sister had a heart attack before age 50.
